Patient Forms & First Visit
In order to ensure we have a full understanding of what you’re looking to achieve with your visits to Georgian Dental®, we ask that all new patients complete our new patient information form. This allows us to make sure we’re all on the same page regarding your treatments, and that we’re aware of any pre-existing medical conditions prior to beginning treatments.
Financial Concerns
Costs for dental procedures aren’t cheap these days, at Georgian Dental® we understand that while you might want, or need a procedure, you may not be able to afford it. That’s why we’ve implemented a range of payment arrangements for more complicated and expensive dental procedures like braces, dental implants and gum surgery.
